What is the female equivalent word for "gentleman"?

Back in the day it was "Gentlewoman," meaning a woman who was either of noble birth or who has impeccable manners and is honorable (as you describe), but that fell out of use and the appropriate word now is "Lady." To be a true "lady," one must have all the qualities you described.
